Change of Advisor

This form must be completed by either the student or the student's current advisor before a student's advisor will be changed. When submitted, this form submits directly to the Office of the Director of Advising. The Director will review the form, process the request for the new advisor, and notify the the student, the new advisor, the previous advisor and the Registrar's office via email, upon approval.
